Media irresponsibility or something sinister?

Shakespeare's Sister brings us this story of CNN using the same picture to show nuclear facilities in Iran and North Korea. Is it incompetence or part of some sinister plot? I vote for incompetence but since we are "fair and balanced™" here at MEJ you check it out and you decide.
